MR Number: 1-95-0015 Nine Mile Point 2 Date: 02/01/95 Lycoming,New York SRI PC Dockets: 50-410 BWR/GE-5 Subject: INOPERABLE EMERGENCY DIESEL GENERATORS (UPDATE) Reportable Event Number: 28304 Discussion: On January 29, 1995, NMPC commenced a shutdown of Unit 2, and declared an Unusual Event as a result of both Emergency Diesel Generators (EDG) being inoperable. On January 26, 1995, during routine monthly surveillance testing, the Division I EDG exhibited instability in the engine control system and tripped when its output circuit breaker was tripped. Troubleshooting of the control system indicated a problem with the electric governor or the DC governor power supply (the engines are equipped with both electric and mechanical governors). The applicable Technical Specification (TS) limiting condition for operation (LCO) was entered, and the Division II EDG was tested. No instability was noted during the run of the Div II EDG. After replacing the Div I EDG electric governor control card, the Div I EDG was tested, and still exhibited instability. During subsequent troubleshooting, excessive temperatures were noted at the base of the governor, and it was determined that the mechanical governor was malfunctioning. The mechanical governor was replaced and on retest, the engine was noted to operate properly, but the governor was still experiencing excessive temperatures. The Div II EDG was run to obtain temperature profiles on its governor, and it was determined that its governor was also experiencing excessive temperatures. At the end of the test run, the Div II EDG failed to trip due to an air leak on the pneumatic controls. The Div II EDG was declared inoperable and a reactor shutdown was commenced (the Div II EDG was INOP, but available). NMPC has determined that the governor coolers were receiving inadequate flow from the jacket cooling water system, and has instituted a design change to rectify the problem. NMPC has also determined that the manufacturer (Cooper-Bessemer) has used several different jacket cooling water system designs, and notified the two facilities with similar systems (Byron and Braidwood) of the problem. The governor on the Div II EDG has been replaced due to having been subjected to excessive temperatures. The problem is believed to be long-term degradation of the governor oil and mechanical components due to the high temperatures. The governors from both engines will be sent out for failure analysis. As of 8:00 a.m. February 1, the modifications on both engines have been completed, and testing of the Div II EDG is in progress (Div I EDG testing is complete). The condition is being reviewed by NMPC for reportability under 10 CFR 21. Regional Action: The resident inspectors are continuing to follow the progress of the work. Davis, the President and Chief Operating Officer for the Boston Edison Company, announced that he will be retiring on September 1, 1995. Mr. Davis joined BECo as a Vice President in 1989, after retiring from the Navy with the rank of Vice Admiral. His initial responsibilities in the company related to the nuclear administration and operation of the Pilgrim Nuclear Power station. In July 1994, he became BECo President and COO. BECo Chairman and Chief Executive Officer Tom May indicated that a successor to Mr. Davis has not yet been named, but that the search will extend beyond the electric utility industry. In announcing his retirement at this time, Mr. Davis also indicated that the next seven months should allow sufficient time for selecting an experienced replacement and facilitating the planned transition. MR Number: 1-95-0017 Limerick 2 Date: 02/01/95 Philadelphia,Pennsylvania SRI Fax Dockets: 50-353 BWR/GE-4 Subject: MOMENTARY LOSS OF SHUTDOWN COOLING SYSTEM Reportable Event Number: N/A Discussion: On January 28, 1995, with Unit 2 in the Refueling mode, the B RHR service water pump automatically tripped. At the time of the trip, the B RHR service water pump was providing the ultimate heat sink for decay heat removal with the B RHR pump in the shutdown cooling mode; the reactor vessel head was in place and detensioned. Main control room operators immediately identified the tripped pump, and after verifying that no other system problems had occurred, restored the RHR service water system by starting the D pump; this was completed within three minutes of the trip. No change in coolant temperature was observed, and the RHR system remained in service throughout the time period. Additionally, the redundant loop of RHR, including the redundant loop of RHR service water was unaffected and was available at that time. Investigation could not determine the cause of the tripped breaker, but it was not due to personnel error. The pump was subsequently successfully tested, and returned to service. Regional Action: The resident inspectors reviewed the event and concluded that actions taken were timely and appropriate. Date: 02/01/95 San Onofre 2 3 Senior Resident Inspector San Clemente,California Dockets: 50-361,50-362 PWR/CE,PWR/CE Subject: UNESCORTED ACCESS GRANTED FOLLOWING POSITIVE DRUG TEST Reportable Event Number: 28309 Discussion: On January 30, 1995, the licensee granted unescorted access to the San Onofre Protected and Vital Areas to a contractor whose initial drug screening results had not been received. At the time of granting the individual unescorted access, the licensee had not completed the drug screening confirmation section of the access authorization form. On January 31, 1995, the drug screening results (positive for marijuana) were received, and in attempting to complete the access authorization form, the licensee discovered that unescorted access had already been granted. The licensee immediately located the individual and terminated his access. The licensee determined that the individual had been in the protected area for approximately one hour and that he had been under direct surveillance by his supervisor for all but about 10 minutes of that time.
