NRC issued this bulletin to (1) inform all radiography licensees about the failure of some licensees to have an NRC-approved quality assurance program for transportation of radioactive material, (2) request all radiography licensees to review their operations and implement quality assurance requirements as necessary, and (3) require all radiography licensees to respond to questions in the bulletin. MR Number: 3-95-0005 Fermi 2 Date: 01/18/95 Newport,Michigan Dockets: 50-341 BWR/GE-4 Subject: FERMI 2 RESTART - FINAL UPDATE Reportable Event Number: N/A Discussion: ON JANUARY 18, 1995, AT APPROXIMATELY 7 P.M., THE FERMI 2 GENERATOR OUTPUT BREAKERS WERE CLOSED, ENDING THE OUTAGE THAT BEGAN WITH THE TURBINE FAILURE EVENT OF DECEMBER 25, 1993. THE PLANT IS CURRENTLY AT 147 MEGAWATTS ELECTRIC OUTPUT. AT THE TIME OF SYNCHRONIZATION, TURBINE VIBRATION ROSE SLIGHTLY TO 9.4 MILS ON BEARING NUMBER SIX, BUT HAS SUBSEQUENTLY DECREASED SLIGHTLY. VIBRATION ON SEVERAL OF THE OTHER TURBINE BEARINGS IS SLIGHTLY HIGH, RANGING FROM 6.1 MILS TO 8.6 MILS. THE LICENSEE PLANS TO COMPLETE EXCITER TESTING OVER THE NEXT TWO DAYS, RAISE POWER TO APPROXIMATELY 65 PERCENT TO FURTHER EVALUATE POWER CHANGE EFFECTS ON VIBRATION, AND THEN SHUT THE TURBINE DOWN TO PERFORM TURBINE BALANCING. MR Number: 3-95-0006 Callaway 1 Date: 01/19/95 Fulton,Missouri Dockets: 50-483 PWR/W-4-LP Subject: PLANT CLOSURE DUE TO INCLEMENT WEATHER Reportable Event Number: N/A Discussion: The Callaway Plant was closed today for non-essential personnel due to heavy snow in the area overnight. Essential personnel are on site and all work which could result in a plant transient has been halted. The licensee has evaluated the impact of this storm as it relates to their Emergency Plan and no problems were noted. The snow is expected to end this evening with a total accumulation of 8 to 12 inches. Regional Action: Region III will continue to monitor the licensee's actions. MR Number: 3-95-0007 Byron 1 2 Date: 01/18/95 Byron,Illinois Dockets: 50-454,50-455 PWR/W-4-LP,PWR/W-4-LP Subject: CONTRACTOR PERSONNEL DIED AT BYRON STATION OF APPARENT CORONARY Reportable Event Number: N/A Discussion: At approximately 8:00 a.m., on January 18, 1995, the control room received notification that a Babcock and Wilcox contractor was found unconscious at his desk in the temporary contractor office space. The contractor office space are trailers located outside the turbine building, away from any radiological area, but inside the licensee's protected area. The control room dispatched an emergency medical technician (EMT) and radiological protection personnel to the scene. An ambulance was also called. The individual was found at his desk, unconscious and not breathing; the individual was NOT contaminated. CPR was administered by the EMT until the ambulance and paramedics arrived at the scene. The individual was transported to Rockford Memorial Hospital, where he was pronounced dead at approximately 9:17 a.m.. The apparent cause of death was a coronary.
