The licensee indicated that a routine incoming contamination survey of the surface of the package revealed contamination levels of 525,472 disintegrations per minute per 100 square centimeters. The licensee promptly evaluated the contents of the package and determined that the package was not leaking. Through the use of a multichannel analyzer, the licensee identified the contaminant as technetium- 99m (Tc-99m). The licensee performed extensive contamination surveys throughout its facility and identified a few areas of low level contamination which were promptly decontaminated. The licensee indicated that the supplier of the package was Mallinckrodt Medical, Inc., a radiopharmacy in Greenbelt, Maryland. The licensee indicated that radiopharmacy personnel indicated during a telephone conversation that in the morning they had an explosion of a cardiolyte vial containing 600 millicuries of Tc-99m and that, as a result of the confusion generated by the incident, contaminated packages may have inadvertently left the facility for distribution to customers. Regional Action: Region II contacted the Region I Government Liaison Officer, who contacted the State of Maryland. The State of Maryland has dispatched an inspector to the radiopharmacy.
