The driver was seriously injured and remains hospitalized. The courier was transporting radiopharmaceuticals for Syncor Nuclear Pharmacy, Jackson MS. The four transportation containers were ejected from the vehicle, two remained closed, and the remaining two were opened. The primary shield containers from the two boxes were not breached. The shipments were unit doses and bulk containers of radiopharmaceuticals for medical use. The radioisotopes were technetium 99m and iodine 123 capsules. The State of Mississippi Division of Radiological Health and Syncor representatives responded to the scene and retrieved the materials. No contamination was found. Regional Action: The State of Mississippi will complete the investigation of this event. Region II plans no further action in this matter. Contact: C. M. HOSEY (404)331-5614 PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION II JANUARY 6, 1995 Licensee/Facility: Notification: Tennessee Valley Authority MR Number: 2-95-0005 Sequoyah 2 Date: 01/06/95 Soddy-Daisy,Tennessee Dockets: 50-328 PWR/W-4-LP Subject: UPDATE ON SEQUOYAH UNIT 2 TRIP Reportable Event Number: 28209 Discussion: On January 5, 1995, at 12:15 a.m., Unit 2 experienced a turbine trip/reactor trip from 100% due to a trip of the main bank transformer "C" phase. The licensee's post trip review identified the root cause to be associated with a loose light socket that rotated during the licensee's investigation of a control room turbine benchboard indicating light. The indicating light was not lit, which the licensee later found was due to a stuck oil flow indicating switch inside the transformer. During an attempt to remove the lens cover from the indicating light, the light socket rotated. Rotation of the socket allowed the terminals of the socket to come in contact with the terminals of an adjacent socket. The momentary fault caused transformer coolers to be momentarily deenergized. As lens cover removal continued, the cooler control bus voltage returned to normal. This return to normal condition allowed all contactors/relays that had dropped out to become reenergized, which resulted in an immediate restart of the coolers. A wave action was initiated inside the transformer piping system to a gas operated relay (pressure sensor). This relay actuated, resulting in a direct turbine trip, and subsequent reactor trip. MR Number: 3-95-0003 Zion 2 Date: 01/06/95 Zion,Illinois SRI VIA PC Dockets: 50-304 PWR/W-4-LP Subject: UNIT 2 SHUTDOWN FOR REFUELING Reportable Event Number: N/A Discussion: At 12:45 a.m. on January 6, 1994, Unit 2 was taken off line for refueling. The reactor was shutdown at 12:46 a.m. During the shutdown, the rod position indication panel indicated that two rods were not fully inserted and the licensee emergency borated in accordance with their abnormal operating procedures. It was later determined, through voltage readings of the secondary drive coils, that the rods were fully inserted. Additionally, approximately thirty minutes after tripping the reactor, both source range indicators failed to energize. The licensee is currently replacing the P-10 permissive relay which should have allowed the source range indicators to be energized. Major work planned during the outage includes diesel generator inspections and overhauls, incore instrumentation modifications, main generator rotor replacement and motor- operated valve testing. MR Number: 4-95-0002 Arkansas Nuclear 2 Date: 01/06/95 Russelville,Arkansas Senior Resident Ispector Dockets: 50-368 PWR/CE Subject: MIDCYCLE STEAM GENERATOR (SG) INSPECTION OUTAGE Reportable Event Number: N/A Discussion: On January 6, 1995, at 8:30 p.m. (CST), the licensee will begin decreasing power to perform a midcycle SG tube inspection. The licensee will review the condition of the SG tubes because of previously experienced circumferential cracking. The plant is scheduled to restart on January 18, 1995; the SG outage window is 6 1/2 days beginning January 10. The licensee will inspect approximately 5000 tubes in both SGs in the hot leg expansion transition region using a rotating pancake probe. Other activities scheduled to be accomplished include differential pressure testing of 12 motor operated valves, change out of Reactor Coolant Pumps A and C seal packages, replacement of a solenoid-operated steam admission valve with a motor-operated valve to the turbine-driven emergency feedwater pump, and verifying proper actuation of synchronization relays for the station blackout emergency diesel generator. MR Number: 4-95-0003 Diablo Canyon 1 2 Date: 01/06/95 Avila Beach,California Resident Inspectors Dockets: 50-275,50-323 PWR/W-4-LP,PWR/W-4-LP Subject: HEAVY SEA CONDITIONS AFFECTING CIRCULATING WATER SYSTEMS Reportable Event Number: N/A Discussion: Recent winter weather systems have caused heavy sea conditions off the California coast. These Pacific Ocean storms are expected to continue over the weekend. The high seas from these storms have resulted in the fouling of intake screens to the San Onofre and Diablo Canyon circulating water systems. Diablo Canyon Units 1 and 2: Diablo Canyon Unit 2 curtailed power January 5-6 to 50 per cent to clean the condenser water boxes of ingested kelp and red sea grass. Unit 2 is ramping to full power today and is expected to reach 100 percent power at approximately 12 midnight tonight. Unit 1 had reduced power earlier in the week to conduct similar cleaning and is currently at 100 percent power. Both units at Diablo Canyon are currently experiencing heavy loading of kelp and sea grass on the traveling screens at the intake structure. The licensee is currently monitoring the traveling screens closely to ensure their operability. SONGS Units 2 and 3: San Onofre Unit 2 has reduced power to 76 percent due to clogging of the main condenser from the influx of red sea grass due to Pacific Ocean storm conditions. Operation at this reduced power level allows periodic stopping and starting of the circulating water pumps to clear some of the sea grass from the condenser. In addition, the operating travelling rake and screen system is being manually assisted in clearing the sea grass. Unit 3 has been less affected by the influx and continues operation at 97 percent power. Unit 2 will continue to operate through the weekend at 76 percent power due to the forecast of continued Pacific Ocean storms.
