At the completion of the audit, the licensee announced the demotion of the General Supervisors for Electrical Maintenance and Mechanical Maintenance to first line supervisors, and a two week suspension for an electrical maintenance supervisor. The job actions were a result of inappropriate time sheet approvals, improper use of compensatory time, and use of company time and equipment for home projects. The licensee also announced the selection of a replacement Maintenance Manager. Mr. Gerry Waig a senior operator instructor assumes the Maintenance manager position. Mr. Waig has progressed within the licensee's operation department from auxiliary operator to shift supervisor, and the last ten years he has been a licensed operator instructor. Regional Action: Regional and Resident Inspectors are monitoring the performance effects of the recent job actions and management changes. MR Number: 1-94-0111 Quality Biotech, Inc. Date: 10/05/94 Camden,New Jersey Dockets: 03030416 License No: 29-28162-01 Subject: Burglary at Licensee's Facility Reportable Event Number: N/A Discussion: Quality Biotech, Inc. is using NRC licensed materials for research and development. The licensee uses tritium (H-3), Carbon-14 (C-14), Sulfur-35 (S-35) and Iodine-125 (I-125) in small quantities (millicuries) at their Camden, NJ facility. During an NRC inspection at the facility on October 4, 1994, the licensee reported to the inspectors that a break-in and burglary occurred at the facility the night before. At 8:00 p.m. on October 3, 1994, the Alarm Monitoring Company, ADT, informed the licensee that an alarm was received from the facility indicating an open status at the above address. The licensee confirmed that the housekeeping staff was in the building at the time and requested that ADT call the licensee back at 9:30 p.m. if the building was not secured. ADT did not call back the licensee, since they mistakenly thought that the licensee would call them back. A licensee employee arrived at the laboratory at 3:00 a.m. on October 4, 1994, and found that the outer perimeter gate was secured, however, the building glass door was shattered. The employee left the scene and called ADT. ADT did not respond. At 7:15 a.m. the licensee's Vice President arrived and found the broken glass and called the police. Examination of the facility by the licensee and the police indicated that some computer components were stolen. The licensee immediately performed an inventory of all radioactive materials in the building. There was no indication of a break-in in the radioactive materials area and all radioactive materials were accounted for. Regional Action: The NRC will review the licensee's actions to improve security. Contact: Kathy Dolce (610)337-5251 Mohamed Shanbaky (610)337-5209 PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION II OCTOBER 5, 1994 Licensee/Facility: Notification: MR Number: 2-94-0083 Virginia Tech Date: 10/04/94 Blacksburg,Virginia Telephonic to Region II Dockets: 03011313 License No: 45-09475-30 Subject: Missing Shipment of Licensed Material Reportable Event Number: N/A Discussion: On October 4, 1994 NRC Region II received a courtesy telephone call from the Radiation Safety Officer (RSO) at Virginia Polytechnic Institute and State University (VA Tech) that an expected shipment of 5 millicuries (mCi) of tritiated thymidine (H-3) had not arrived. The shipment was expected on August 22, 1994. On September 6, 1994, the VA Tech RSO notified the shipper (DuPont-New England Nuclear) that the package had not been received. The shipper contacted the carrier (Airborne Express) and a trace was put on the shipment. On September 12, the VA Tech RSO was informed by the carrier that the shipment could not be located. On September 27, the Radiation Safety Committee of VA Tech met and decided to inform the NRC of the lost shipment, even though there was no requirement for them to do so. This shipment (5 mCi) is one half of the reportable quantity for H-3 per 10 CFR 20.2201(a)(ii). Regional Action: The Commonwealth of Virginia has been informed.
