MR Number: 3-94-0192 Quad Cities 2 Date: 10/25/94 Cordova,Illinois ens Dockets: 50-265 BWR/GE-3 Subject: UNUSUAL EVENT FOR TRANSPORTING A POTENTIALLY CONTAMINATED INJURED PERSON Reportable Event Number: N/A Discussion: At 7:56 a.m. (CDT) on October 25, 1994, the licensee entered into an Unusual Event for transporting a potentially contaminated injured man offsite via an ambulance. A contract fire watch inspector fell down the ladder to the Unit 2 cable tunnel area. The licensee was unable to survey the individual's back prior to releasing him. A radiation protection technician accompanied the injured person to the hospital. The individual was later surveyed to be not contaminated.
