"Bill" Pearce has accepted the position of Quad Cities Station Manager, and will be relieving Mr Ed Kraft as Station Manager. Mr. Kraft had been filling a dual role as Site Vice President and acting Station Manager since October 14. Mr. Kraft will remain Site Vice President . Mr Pearce will be arriving on site in about 2 weeks. He was formerly a 27 year employee of Florida Power and Light, where his most recent assignment was Plant General Manager for Turkey Point. MR Number: 4-94-0131 Comanche Peak 2 Date: 11/05/94 Glen Rose,Texas Senior Resident Inspector Dockets: 50-446 PWR/W-4-LP Subject: CONTAINMENT SUMP SCREEN BYPASS HOLES Reportable Event Number: 27997 Discussion: This morning report provides an update to the 10 CFR 50.72 report made by the licensee on November 5, 1994, regarding the discovery of gaps around the containment sump screens on Unit 2. On Friday, November 4, 1994, the licensee performed a walkdown of the two CPSES Unit 2 containment sump screen structures. The walkdown was performed because of lessons learned from a review of previous findings at South Texas Project where screen bypass holes were discovered and reported in January 1994. CPSES Unit 2 is currently in its first refueling outage. The licensee's inspection of the two Unit 2 containment sump screen structures revealed a total of 48 bypass holes with varying dimensions. The largest dimension was approximately 5/8 inch by 1-1/4 inch. The licensee requested an analysis of the Unit 2 findings from Westinghouse and on Saturday, November 5, 1994, the license declared all Unit 2 Emergency Core Cooling Systems inoperable pending completion of the analysis by Westinghouse. Westinghouse subsequently completed the analysis of the Unit 2 containment sumps and based on the analysis the licensee concluded that the Unit 2 containment sumps were operable with the existing gaps. On Saturday evening, the licensee performed a Unit 1 containment entry with the unit at 100% power to inspect the containment sump screen structures to determine if any gaps existed in Unit 1 screen support structures which were not bounded by the Westinghouse analysis. The licensee found similar screen bypass holes in Unit 1 as in Unit 2, however, none exceeded the analysis assumptions provided by Westinghouse. Regional Action: The resident inspector responded to the site on November 5, 1994, and Region IV monitored licensee actions to inspect Unit 1 screens.
