MR Number: 3-94-0206 Zion 1 Date: 11/02/94 Zion,Illinois Dockets: 50-295 PWR/W-4-LP Subject: SHUTDOWN FOR FORCED OUTAGE; UPDATE Reportable Event Number: N/A Discussion: Following repairs for a Number 11 main generator exciter bearing lube oil leak, Unit 1 was taken critical at 11:39 p.m. on November 4, 1994. While rolling the main turbine at 3:20 a.m. on November 5, a megger reading on the Number 11 bearing pedestal indicated the pedestal (and the generator) to be grounded. Based on discussions with Westinghouse, Unit 1 was taken to Mode 4 to facilitate investigation of the grounded pedestal. Currently, the pedestal is being disassembled and inspected. MILLER (708)829-9629 PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION IV NOVEMBER 7, 1994 Licensee/Facility: Notification: Irt Corporation MR Number: 4-94-0130 Irt Corporation Date: 11/04/94 San Diego,California Dockets: 07001359 License No: SNM-1405 Subject: PuBe SOURCE SHIPPED IN VIOLATION OF DOT REGULATIONS Reportable Event Number: N/A Discussion: The licensee's Radiation Protection Officer (Kay Crosbie) notified the Region IV WCFO at approximately 1:30 p.m. on November 4, 1994, that the licensee had been informed by the DOE laboratory at Los Alamos, New Mexico, that a PuBe source IRT that had shipped to them was shipped via nonexclusive-use mode when it should have been shipped exclusive-use. Since the shipment was not made exclusive-use, IRT did not survey the cab of the tractor/trailer rig to confirm that dose rates were less than the required 2 mrem/hr in the normally occupied area of the tractor. DOE LAL also informed IRT that the dose rate at 1 meter from the PuBe source package was determined by them to have a combined gamma+neutron dose rate of 26 mrem/hr at 1 meter, and not the 11 mrem/hr indicated by IRT on the shipping papers. IRT indicated that when the tractor/trailer rig of Consolidated Freight Co. left the IRT facility, the PuBe package (DOT spec 6M 30-gallon drum) was on the extreme end of the trailer away from the tractor cab, 15 or more feet distance from the cab. Regional Action: Region IV WCFO has requested the licensee to verify the route taken by the material and determine dose estimates of any truck drivers involved in its routing to DOE LAL from San Diego, CA.
