MR Number: 1-94-0121 Calvert Cliffs 2 Date: 11/02/94 Lusby,Maryland SRI/TELEPHONE CALL Dockets: 50-318 PWR/CE Subject: GENERATOR DAMAGE Reportable Event Number: N/A Discussion: Unit 2 experienced a turbine trip and reactor trip late in the evening of August 27, 1994 (Events No. 27961 and PNI-9463) as a result of an electrical fault in the main generator. BGE found that the links which connect the generator phase "A" to the isolated phase bus are severely damaged. The BGE investigation into the problem has determined that the links were installed backwards (with the painted side against the contact surfaces rather than the silver-plated side) during the last refueling outage. It is postulated that this resulted in high resistance and overheating at the connections. Repair parts have been ordered, and are expected to arrive by the end of the week. BGE intends to maintain the plant in hot standby until repairs are completed. Regional Action: The resident inspectors will continue to monitor the BGE investigation and recovery. Contact: R. MR Number: 3-94-0198 Point Beach 2 Date: 11/02/94 Two Rivers,Wisconsin sri Dockets: 50-301 PWR/W-2-LP Subject: unit 2 startup following refueling outage Reportable Event Number: N/A Discussion: Unit 2 was started up following completion of refueling outage 20 and the main generator was placed on line November 1, 1994. This 39-day outage included installation of a new emergency diesel generator (EDG). The station now has three operable EDGs, one is a dedicated "A" train for both units and the other two are dedicated one per unit for "B" train. In addition, during steam generator eddy current testing, the licensee utilized the CECCO-5 probe to inspect previously sleeved tubes for indications. Based on the indications found a total of 167 tubes in both generators were plugged, 155 of which were sleeved tubes with circumferential indications in the parent tube. MR Number: 3-94-0199 Dresden 3 Date: 11/01/94 Morris,Illinois RI Dockets: 50-249 BWR/GE-3 Subject: START UP OF UNIT 3 AFTER EXTENDED REFUELING OUTAGE Reportable Event Number: N/A Discussion: On November 1, 1994, at about 9 p.m. (CST), Unit 3 was taken critical and is expected to be synchronized to the electrical grid on November 4. The unit was shut down on March 9, 1994, for the thirteenth refueling outage. On August 5, 1994, at about 0530 (CST), Unit 3 was taken critical; however, on August 6 the reactor automatically shut down due to low reactor water level when control room personnel failed to monitor the reactor level while adjusting the condensate system. Prior to the November start-up, the licensee implemented numerous initiatives to improve operator and system performance, reduce operator workarounds, and reduce plant contamination. One specific initiative was to "unitize" the operations and maintenance departments to provide more management attention on station problems.
