The operator was withdrawing the rod from the full in position (00) to the full out position (48) when a rod block occurred at position 20 due to high neutron flux. When the rod block cleared, the operator resumed continuous rod withdrawal at which time he noticed the rod inserting. He immediately released the control switches and the rod settled at position 04. General Electric stated that during continuous rod withdrawal, in rare cases, the rod may insert if the notch override and notch out switches are engaged simultaneously. The licensee was not able to repeat the occurrence and they also ruled out operator error. On November 22, 1994, during control rod surveillance testing, an operator provided a continuous rod withdrawal signal to a rod while it was at position 48 for a coupling check. The rod did not move (since the rod was coupled), but both the rod in and rod out travel lights illuminated simultaneously. The licensee was able to repeat this occurrence with 13 other rods. Troubleshooting by Instrumentation and Controls identified, on several occassions, the simultaneous, or near simultaneous energization of the "unlatch" and "rod withdrawal" relays. Per GE drawings there should be a .06 second delay between them. A spare timer was tested and found to have the approriate delay. NYPA plans to replace the malfuntioning solid state timer in the Reactor Manual Control System during the upcoming refueling outage. MR Number: 1-94-0128 Calvert Cliffs 1 Date: 11/25/94 Lusby,Maryland SRI PC Dockets: 50-317 PWR/CE Subject: CRACKED CHARGING PUMP CASING Reportable Event Number: N/A Discussion: On Thursday, November 24, 1994, operators at the plant noticed water around the 11 charging pump. The system Engineer was called in to investigate the cause, and determine corrective action. The initial determination was that the apparent cause of the leakage was a cracked pump casing. The pump is currently isolated, and tagged out of service. The remaining 2 positive displacement pumps meet the technical specification requirements for high head safety injection. Investigation of the pump condition and corrective actions will continue on Monday, November 28, 1994, when the company returns from holiday. Of particular concern to the staff was the lack of safety sensitivity demonstrated when licensed operators knowingly operated the makeup tank outside of the region permitted by procedures, licensee personnel made untimely operability determinations for certain RPS trip setpoints, and technicians arbitrarily set the ATWS-AMSAC setpoints resulting in system inoperability. MR Number: 4-94-0134 Comanche Peak 2 Date: 11/25/94 Glen Rose,Texas Senior Resident Inspector Dockets: 50-446 PWR/W-4-LP Subject: UNIT 2 RESTART FOLLOWING REFUELING OUTAGE Reportable Event Number: N/A Discussion: Following the first refueling outage, Unit 2 was taken critical at 6:01 p.m., on November 22, 1994, and initially synchronized to the grid on November 24 at 4:56 a.m. As Unit 2 reached 17 percent power, the main turbine tripped from high primary water (main generator cooling) temperature when the temperature increased to the trip setpoint while the operators were controlling the cooling system in manual control. Approximately 1 hour later, Unit 2 was resynchronized to the grid. The licensee's original outage schedule called for synchronizing to the grid on November 21, 1994. Major activities accomplished during the outage included steam generator eddy current testing, diesel generator and main generator inspections, reactor coolant pump inspections and seal replacements, containment spray system modifications, and electrical bus outages.
