MR Number: 3-94-0220 Dresden 2 Date: 11/22/94 Morris,Illinois Dockets: 50-237 BWR/GE-3 Subject: UNIT 2 START-UP AND SHUT-DOWN. NEW STATION VICE PRES. ONSITE Reportable Event Number: N/A Discussion: On November 19, 1994, at about 11:00 a.m. (CDT) Unit 2 was taken critical. However, on November 20 the licensee identified a steam leak on an elbow of a 3/4 inch line off the 'B' main steam line and reinserted all rods at about 7:00 p.m. The leak caused a tear in the X-area to turbine building penetration boot. The licensee started the standby gas treatment system and verified secondary containment operability. The licensee plans to repair the boot seal and pipe and continue the start up on November 23. Mr. Thomas Joyce arrived on site November 21, 1994 and will take over as the new station vice president in December.
