NU management intends to curtail the majority of work until adequate corrective actions for improper work control can be implemented. Several recent problems, including the Unusual Event (event #27930) from saltwater spray of the 4160 volt switchgear, prompted this management action. Interim work that will be allowed to proceed includes offloading all fuel, placing the steam generators in wet layup, and implementing adequate freeze protection. Additionally, certain main generator work and required surveillances will be conducted. The majority of outage contractors are being released and the site staff will return to their regular 40 hour work week. The overall impact on the outage schedule is not certain at this time. MR Number: 3-94-0197 Zainsville Fire Dept. Date: 11/01/94 Zainsville,Ohio TELECON W/ GOOD SAMARITAN HOSPITAL Subject: RADIATION ALARM ON INCOMING SHIPMENT Reportable Event Number: N/A Discussion: Region III was notified that a shipment of waste caused a radiation monitoring system to alarm at an Ohio waste landfill on October 27, 1994. The contents were segregated from other shipments and secured until the Zainsville, Ohio fire department took possession of it. The fire department segregated the material and placed it in a secure area. A local physicist from Good Samaritan Hospital was contacted by the fire department and he measured 4 mR/hr (4 E -2 mSv/hr) at the surface with a GM survey meter. It was determined that the increased radiation levels were from two plastic bags containing paper towels, facial tissue and cigarette butts. It could not be determined where the material originated. Regional Action: Region III referred this matter to EPA for followup in accordance with the draft Federal Radiological Response Plan. NMSS has been notified as well as the Ohio Department of Health.
