The employee's plant access has been suspended pending the results of the matter being processed through the licensee's FFD appeal process. The outage was due to an oil leak on the exciter skid provided by Westinghouse. During the outage, the licensee repaired the oil leak and addressed a potential ground on the exciter. MR Number: 3-94-0215 Duane Arnold 1 Date: 11/15/94 Palo,Iowa RIA VIA LAN Dockets: 50-331 BWR/GE-4 Subject: REACTOR SHUTDOWN DUE TO FORCED OUTAGE Reportable Event Number: N/A Discussion: On November 15, 1994, a controlled reactor shutdown to cold shutdown is planned to commence at 10:00 p.m. (CST) due to an increase in unidentified drywell leakage. Unidentified drywell leakage increased from approximately 0.54 gallons per minute (gpm) on November 7 to 0.80 gpm on November 11 to 1.17 gpm on November 15. The technical specification limit for unidentified leakage is 5 gpm. The outage is expected to last for 5 days, with start-up planned for Sunday, November 20, 1994. The cause of the drywell leakage is unknown at this time, however, the licensee suspects packing leakage from a main steam isolation valve or the reactor core isolation cooling system inboard steam isolation valve at this time. The plans for the outage are still being finalized, depending on the cause of the drywell leakage and the repair time. The final forced outage list of other items to be repaired will be determined later today. MR Number: 3-94-0216 Braidwood 2 Date: 11/16/94 Braceville,Illinois SRI Dockets: 50-457 PWR/W-4-LP Subject: UNIT 2 RESTART FOLLOWING REFUELING OUTAGE Reportable Event Number: N/A Discussion: On November 16, 1994, at 0350, Unit 2 went critical following a 40- day planned refueling outage. In addition to complete core offload and refueling, major activities accomplished during the outage included safety- related battery replacements, steam generator eddy current testing, 2B/2C reactor coolant pump inspections and seal replacements, and a containment integrated leak rate test (ILRT). The licensee plans to synchronize to the grid following core physics testing which should be completed within 48 hours. Regional Action: The resident staff will continue to monitor plant restart.
