MR Number: 3-94-0208 Dresden 3 Date: 11/09/94 Morris,Illinois ri Dockets: 50-249 BWR/GE-3 Subject: CONTROL ROD FAILED TO SCRAM DURING SCRAM TIME TESTING Reportable Event Number: N/A Discussion: On November 8, 1994, while performing control rod scram time testing at power, control rod drive E-10 failed to scram. The apparent cause of the failure was a malfunction of one or both of the scram pilot solenoid valves. The solenoid valves are connected in series and both must actuate to vent the air pressure from the scram inlet and outlet valves. The solenoids for this rod were rebuilt earlier this year including diaphragm and core assembly replacement. The manufacturer, Automatic Switch Company, will perform an investigation into the cause of the failure at their Florham Park, New Jersey, facility. The results of the investigation are expected on November 10. A licensee representative will assist the investigation. As of 0700 on 11/9/94, the licensee has completed 113 of 177 rod scram tests. While cleaning out a house, an individual discovered, four black/silver square shaped metal rods. Each rod was about two inches long, with the thickness of a pencil, pointed on at least one end, weighing about the same as a roll of nickels, and labeled "electrolyte uranium". The rods were discovered with screws and other types of hardware in a wooden cabinet in the basement of the house. The individual was instructed to keep the rods in storage. On November 9, 1994, radiation surveys and wipe tests for contamination will be performed by a representative of the Ohio Department of Health at the individual's house. Regional Action: NMSS, EPA, and the Ohio Department of Health were notified. MR Number: 4-94-0132 Diablo Canyon 2 Date: 11/09/94 Avila Beach,California RESIDENT INSPECTOR Dockets: 50-323 PWR/W-4-LP Subject: EVIDENCE OF FUEL PIN LEAKAGE Reportable Event Number: N/A Discussion: Unit 2 Reactor Coolant System (RCS) dose equivalent iodine has increased to 0.239 microcurie/milliliter (uCi/ml). This increase is indicative of a fuel cladding defect. The Technical Specification limit for RCS dose equivalent iodine is 1.0 uCi/ml. On November 5, 1994, RCS dose equivalent iodine was 4.03xE-4 uCi/ml. Licensee review of chemistry data indicated that the increase in RCS activity started slowly on November 7, 1994, and increased more rapidly on November 8, 1994. Although the increase in RCS dose equivalent iodine has not stopped, the rate of increase has slowed. The licensee is evaluating the situation regarding further characterization of the possible extent of fuel cladding degradation. The resident inspector is following the issue.
