MR Number: 3-94-0230 Fermi 2 Date: 12/07/94 Newport,Michigan telecon from resident Dockets: 50-341 BWR/GE-4 Subject: fermi 2 readiness for restart - update Reportable Event Number: N/A Discussion: THE LICENSEE HAS REMOVED DEBRIS, FINISHED WORK TO REPLACE MIXED COMPRESSION FITTINGS, AND HAS CORRECTED THE CONFIGURATION CONCERNS WITH THE PIPE SUPPORTS AND CABLES. THE LICENSEE PROJECTS READINESS FOR DRYWELL CLOSURE FOR THIS AFTERNOON. THE NRC AND REGION III MANAGEMENT ARE CLOSELY MONITORING THE LICENSEE'S ACTIVITIES IN THE DRYWELL. NRC DRYWELL INSPECTION WILL BE PERFORMED UPON COMPLETION OF THE LICENSEE'S ACTIVITIES. THE LICENSEE PLANS TO ENTER MODE 2 UPON COMPLETION OF THE DRYWELL CLOSEOUT; HOWEVER, RESTART OF THE PLANT WILL NOT OCCUR UNTIL THE LICENSEE'S RESTART CERTIFICATION CHECKLIST SIGNOFFS ARE COMPLETE. MR Number: 3-94-0231 Dresden 2 Date: 12/07/94 Morris,Illinois resident inspectors Dockets: 50-237 BWR/GE-3 Subject: unit 2 start up from forced outage Reportable Event Number: N/A Discussion: On December 4, 1994, at about 2:00 a.m. Unit 2 was made critical and was synchronized to the grid on December 5 at about 8:20 p.m. Unit 2 was shut down on August 9, 1994, to repair degraded HPCI isolation valves and main steam line drain piping. In addition to the hardware fixes during the shut down, the licensee initiated improvement efforts in overall plant operation including evaluation of operator standards, removal of operator work arounds, and plant cleanup. MR Number: 3-94-0232 Zion 2 Date: 12/07/94 Zion,Illinois ri via pc Dockets: 50-304 PWR/W-4-LP Subject: diesel generator update Reportable Event Number: N/A Discussion: On December 6, 1994, Zion Station made a 50.72 non-emergency one- hour report regarding the concurrent inoperability of the 0 and 2A diesel generators (DG) for a period of approximately 20 hours. In their notification, the licensee stated the 0 DG was inoperable due to a control switch malfunction. This switch controls the starting of the 0 DG ventilation fan and would not have precluded the starting of the DG upon demand. The light bulb malfunction on the 2A DG involved the indication for "engine running" on the DG control panel. The licensee initially believed that with this bulb shorted the diesel start circuitry would be interrupted. Therefore, the DG was declared inoperable. Further investigation determined that the "engine running" indication was not integral to the start circuitry; therefore, the 2A DG would also have started upon demand.
