MR Number: 2-94-0108 Farley 2 Date: 12/19/94 Ashford,Alabama Dockets: 50-364 PWR/W-3-LP Subject: FOLLOWUP TO REACTOR TRIP ON DECEMBER 18, 1994 Reportable Event Number: 28157 Discussion: A reactor trip from 100 percent power occurred when all of the main turbine governor valves went fully closed. The closure of the valves resulted in level shrinkage in all three steam generators. According to the first out annunciator, the reactor trip was generated by "2C" steam generator low-low level (17 percent). All rods fully inserted and all systems functioned as required. The trip was caused by to a malfunction of the auto-stop oil pressure switch that closed the governor valves, causing a level shrink in the S/G that gave a low level trip at +13 seconds. The pressure switch was replaced and the reactor was made critical at 5:06 a.m. (CST), December 19, 1994. At 8:30 a.m. (CST), the reactor was at 10 percent power, and the licensee was preparing to roll the turbine. The Resident Inspector responded promptly to the site, observing licensee corrective actions and the startup. Girvin to vice president of human resources to replace Mr. Robert E. Rigsby who was promoted to senior vice president for finance and controller. Mr. Michael R. MR Number: 3-94-0243 Byron 1 Date: 12/19/94 Byron,Illinois Dockets: 50-454 PWR/W-4-LP Subject: restart from a forced outage Reportable Event Number: N/A Discussion: The Byron Unit 1 was restarted after a 13 day forced outage due to a condenser tube leak of approximately 20 gpm. The reactor was taken critical at 2:59 a.m. (CST) and the generator output breakers were closed at 9:30 a.m. (CST) on 12/16/94. The leak was caused by a small section of condenser deflection grating that broke loose and fell to the bottom of the condenser. The grating damaged two condenser tubes. One tube was only dented, while the other tube suffered a hole of approximately 1 to 2 inches in diameter. The secondary side and steam generator chemistry peaked to the values of 80,000 ppb sulfates, 2500 ppb sodium, and 11,000 ppb chlorides. The secondary side and steam generators were drained and flushed until the chemistry was within acceptable levels for startup. The sulfate ion was the most limiting of the chemicals to clean up. The startup requirement for sulfates was less than 100 ppb. The licensee's root cause team determined that the grating failed due to weld failure. The damaged grating was only secured by five weld points attached to the vanes of the grating. Four of the welds had failed and the grating broke loose when the last vane holding the grating broke due to fatigue failure. The licensee determined that this piece of grating was inadequately secured in the condenser. A thorough inspection of all gratings was conducted and no other discrepancies were identified. The licensee indicated that the extent of damage to the steam generator tubes, caused by the chemical incursion from the condenser tube leak, could not be evaluated until the mid-cycle outage to perform the steam generator tube inspections. The mid-cycle outage is scheduled for September of 1995. Due to the level of sulfate chemistry hideout return, the plant was limited to operation less than 30% reactor power. Subsequent to the reactor startup, the sulfate concentration was approximately 250 ppb. To continue in high power operations, the sulfate concentration must be less than 20 ppb. As of 5:00 a.m. (CST) December 19, 1994, the sulfate concentration was 53 ppb. Regional Action: The resident inspectors observed the licensee's startup activities. The inspectors also followed the licensee's repair activities to the main condenser and steam generator clean up activities. MR Number: 3-94-0244 Davis Besse 1 Date: 12/17/94 Oak Harbor,Ohio by phone to residence Dockets: 50-346 PWR/B&W-R-LP Subject: accidental disharge of a firearm Reportable Event Number: N/A Discussion: At about 3:00 a.m. while on an interior compensatory post (door between the low level radwaste storage building and auxiliary building), a security officer removed his .38 caliber revolver from its holster contrary to procedures. He emptied the cylinder and mistakenly thought he saw six rounds in his hand. He began cleaning the weapon and when completed, pulled the trigger to verify operability. The weapon discharged, striking a door jam nearby. There was no other damage and on one was injured. The security officer was immediately relieved of duty and suspended pending the licensee's continuing investigation. The individual was given a for-cause fitness-for- duty test, the alcohol portion of which was negative. The results of the drug screening will be available during the week of 12/19/94.
