STAREFOS (404)331-5568 PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION II DECEMBER 16, 1994 Licensee/Facility: Notification: Tennessee Valley Authority MR Number: 2-94-0107 Sequoyah 1 2 Date: 12/16/94 Soddy-Daisy,Tennessee Dockets: 50-327,50-328 PWR/W-4-LP,PWR/W-4-LP Subject: FREON LEAK IN MAIN CONTROL ROOM Reportable Event Number: N/A Discussion: On December 15, 1994, at approximately 2:00 p.m., a fire protection technician identified a freon leak coming from a main control room (MCR) chiller located in an adjacent equipment room. This equipment is located inside the MCR envelope. The Shift Supervisor was immediately notified, and decided to evacuate all non-essential control room personnel. One MCR breezeway access door from the MCR to the turbine building was opened, and portable exhaust fans were placed in service to exhaust the MCR. Appropriate security was maintained for this activity. The freon leak was isolated in approximately one hour from the time of discovery. No personnel were physically affected by the freon leak nor impaired from performing their duties. The breezeway door was placed in the normal configuration at approximately 4:00 p.m. The Shift Supervisor determined that entry into the Radiological Emergency Plan was not appropriate. Additional sampling of the MCR envelope determined that no personnel hazard existed. MR Number: 4-94-0146 River Bend 1 Date: 12/16/94 St Francisville,Louisiana Senior Resident Inspector Dockets: 50-458 BWR/GE-6 Subject: DIESEL GENERATOR PLACED ON GRID WITHOUT SYNCHRONIZING Reportable Event Number: N/A Discussion: On December 14, 1994, at 2:35 p.m., during the fast load operational surveillance testing of the Division III diesel generator (DG), the operator closed the DG output breaker to connect the machine to the energized safety bus without synchronizing the electrical buses. The DG immediately tripped on reverse current. At River Bend Station, the Division III DG supplies emergency power to the high pressure core spray pump and supporting components. The degree of non-synchronization was not known; however, the system engineer was in the DG room and did not observe any perturbations other than the DG shutting down. The diesel is a Stewart and Stevenson, Electro Motive Division V-20 and the generator was manufactured by the Ideal Electric Company, and rated at 2850 KW. The licensee implemented an inspection and testing plan with assistance from both vendors. In addition, the generator vendor is on site. As of this reporting, the output beaker and associated relays showed no signs of damage, and all electrical testing has been satisfactory, except that the exciter output diodes were degraded and the field collapse selenium rectifier was shorted. In addition, eight of the wedges on the periphery of the field coils appeared to have been disturbed and have small gaps. The licensee is preparing a spare generator for installation in the event the installed generator is found to be inoperative as a result of inspections and tests. The licensee is also taking actions to address the human performance aspect of this event. Regional Action: Routine followup by the resident inspectors. Contact: C. A. MR Number: 4-94-0145 Diablo Canyon 1 2 Date: 12/16/94 Avila Beach,California Resident Inspector Dockets: 50-275,50-323 PWR/W-4-LP,PWR/W-4-LP Subject: ANNOUNCEMENT OF ORGANIZATIONAL CHANGES AND CUTBACKS AT PG&E Reportable Event Number: N/A Discussion: On December 15, 1994, Pacific Gas and Electric Company (PG&E), the licensee for Diablo Canyon Units 1 and 2, announced several organizational changes and corresponding reductions in staff. Included in these changes is a long term plan to reduce the staff in the Nuclear Power Generation business unit (NPG) from 2350 to 2150 employees over a two year period by reducing the number of contract workers, lay offs, attrition, and early retirement programs. Organizational changes include consolidation of the I&C and Electrical maintenance organizations, consolidation of several engineering functions on site to report to a single manager, reporting to the corporate office, consolidation of the maintenance and outage planning functions, consolidation of several corporate engineering functions, and several other less significant organizational changes. The licensee does not intend to eliminate any functions currently performed by the Nuclear Power Generation business unit.
