The new Millstone 3 Unit Director will be Mike Brothers, presently the Unit 3 Operations Manager. Pete Richardson, the Training Director at Seabrook, will succeed Mr. Brothers as the Unit 3 Operations Manager. Regional Action: Information only. MR Number: 3-94-0238 Cook 2 Date: 12/13/94 Bridgman,Michigan TELECON FROM RESIDENT OFFICE Dockets: 50-316 PWR/W-4-LP Subject: UNIT 2 RESTART FROM REFUELING OUTAGE Reportable Event Number: N/A Discussion: At about 8 a.m. on December 10, 1994, Unit 2 went critical following a refueling outage. At 3:14 p.m. on December 11, 1994, the licensee paralleled Unit 2 to the grid. The outage commenced on September 6, 1994, and the restart was delayed several times due to the following: reactor coolant pump motor replacement, weld repairs to reactor vessel head penetration guide funnels, steam generator manway gasket replacements, and repair of residual heat removal suction valve 2-IMO-128. Major activities during the outage included major control room habitability enhancements, reactor protection system analog to digital change-out, large bore pipe support modifications, conoseal assembly modifications, erosion/corrosion inspection and replacement of secondary piping, and inspection and refurbishment of one of the low pressure turbine rotors. At 5:06 p.m. on December 11, 1994, Unit 2 experienced a reactor trip due to a main turbine trip. The turbine trip was caused by a high level in the moisture separator reheater (MSR) drain tank. The licensee restarted the unit and is continuing to monitor the MSR drain tank level. Regional Action: The resident inspectors will monitor the startup progress. Contact: WAYNE KROPP (708)829-9633 PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION III DECEMBER 13, 1994 Licensee/Facility: Notification: Syncor Coporation MR Number: 3-94-0239 Swartz Creek Facility Date: 12/06/94 Swartz Creek,Michigan Dockets: 03019823 License No: 21-21141-01MD Subject: RADIOPHARMACY CONTAMINATION Reportable Event Number: N/A Discussion: At 5:42 a.m., December 6, 1994, a Pharmacist on duty reconstituted a Cardiolite vial, lot #3594MK, with 545 mCi (millicuries) of technetium-99m and placed the vial into the heating block. After approximately seven minutes of heating of the vial, the Pharmacist noted the room monitors were indicating increased background activity and discovered that the bottom of the Cardiolite vial had broken off during the process. The contents of the vial had leaked out into the heating block and evaporated producing general contamination inside the fume hood and the floor immediately in front of the hood. The Pharmacist and 2 technologists who helped in shielding and isolating the contamination discovered their lab coat sleeves contaminated as a result of their actions. Radiation exposure to the individuals has been estimated by the licensee management as 50-100 millirem due to the release.
