MR Number: 3-94-0235 Perry 1 Date: 12/09/94 Perry,Ohio sri via pc Dockets: 50-440 BWR/GE-6 Subject: control rod movement capability Reportable Event Number: N/A Discussion: Normal control rod movement capability was lost due to a malfunction of the rod deselect switch at approximately 6 a.m. this morning. Control rod trip capability is unaffected by the malfunction. NRR and Region III staff have concurred in the licensee's assessment that the malfunction has not rendered the control rods inoperable. Although no power changes are planned, recirculation flow would be used to control any needed power changes. The malfunctioning deselect switch is expected to be replaced by late morning. Contact: l.r. greger (708)829-9628 PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION IV DECEMBER 9, 1994 Licensee/Facility: Notification: Department Of Veterans Affairs MR Number: 4-94-0141 Department Of Veterans Affairs Date: 12/07/94 Los Angeles,California Telephone call from state of CA Dockets: 03001213 License No: 04-00181-04 Subject: REPORT OF RADIOACTIVE MATERIAL AT BIO-HAZARDOUS WASTE SITE Reportable Event Number: N/A Discussion: On Tuesday, December 6, 1994, TCI Thermal Combustion, of Colton, California, a bio-hazardous waste disposal site, identified four 30-gallon plastic barrels of bio-hazardous waste that contained radioactive materials. The barrels came from the Department of Veterans Affairs Medical Center, Los Angeles. TCI reported their findings to the state of California, Department of Health, Radiologic Health Branch, who in turn reported the situation to the NRC Region IV Walnut Creek Field Office. The contaminated material appears to be patient urine bags or urine soaked pads. Dose rates on the barrels ranged from 0.3 to 1.9 millirem per hour (3 to 19 uSv/hr). The licensee has arranged for transport of the 4 barrels back to the medical center and will investigate the circumstances. The licensee is not aware of any media interest.
