MR Number: 2-94-0079 Catawba 2 Date: 09/16/94 York,South Carolina Dockets: 50-414 PWR/W-4-LP Subject: UPDATE - CATAWBA UNIT 2 TRIP Reportable Event Number: 27781 Discussion: This is an update on the September 13, 1994 Catawba Unit 2 overpower differential temperature (OP delta T) reactor trip which occurred when the C main steam isolation valve (MSIV) went closed. Investigation revealed the cause of MSIV C going closed to be attributable to the failure of a Cutler-Hammer type M relay in the MSIV control circuit. Resolution of other problems encountered during recovery activities included: (1) auxiliary feedwater system reset difficulties were demonstrated to be a function of holding the reset button down long enough for the associated timer circuit to time out; (2) difficulties in reopening the letdown variable orifice and the MSIV C bypass valve were a matter of controller calibration; and (3) the first two steam generators (S/G) C code safeties were confirmed, through testing and S/G pressure traces, to have lifted as required... blockage was found in one of the lift indication pressure sensing lines. Having completed all required startup activities, the unit was taken to Mode 2 (startup) at 2:30 a.m., on September 16, 1994. At 3:00 a.m., with 86 steps on bank C control rods, OP delta T and over temperature differential temperature (OT delta T) alarms were received on Reactor Protection System channel 3. Consequently, the operators returned Unit 2 back to Mode 3 (Hot Standby) in order to investigate the problem. The Licensee's investigation into the cause of the OP delta T and OT delta T alarms is currently in progress. Contamination surveys indicated ground contamination of 730,000 dose per minute (dpm). Bioassays were negative. The licensee will monitor and remediate as necessary. There was no press release. The outage is scheduled to last 57 days. Major activities include high pressure and low pressure Turbine B inspection and refurbishment, 100 percent eddy current testing of Steam Generators A and D, pressure pulse and sludge lance cleaning of all steam generators, replacement of Reactor Coolant Pump C motor, replacement of essential service water containment isolation valves, completion of Generic Letter 89-10 in-plant activities, replacement of the emergency diesel generator exhaust manifolds, elimination of steam generator hydraulic snubbers, and installation of reactor coolant system thermal expansion instrumentation.
