The Radiation Safety Officer (RSO) discovered the loss while performing a routine source inventory on August 25, 1994. The RSO stated that the detectors contained in gas chromatographs may have been inadvertently disposed of with other laboratory equipment that was sent to a local landfill in July 1994. The licensee is continuing its investigation of this incident. The inadvertent disposal of nickel-63 does not represent a significant radiation hazard since it is a low energy beta- emitting radioisotope. The State of Wisconsin and NMSS have been notified of the incident. HOLT (708)829-9836 PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION IV AUGUST 26, 1994 Licensee/Facility: Notification: Washington Public Power Supply System MR Number: 4-94-0086 Washington Nuclear 2 Date: 08/25/94 Richland,Washington Licensee to RI Dockets: 50-397 BWR/GE-5 Subject: INAPPROPRIATE BEHAVIOR INSIDE THE PROTECTED AREA Reportable Event Number: N/A Discussion: On Friday, August 19, 1994, a situation involving inappropriate behavior occurred during a day shift break in the change trailer for the painters located inside the protected area. The event involved three or four people, who had been trading pranks over the last several weeks, taping a temporary painter to a bench, smearing a pie in his face, and putting whipped cream down the back of the painter's trousers. Supposedly, this was in retribution for past pranks in which the individual participated. Maintenance management became aware of the incident on Wednesday morning, August 24, 1994, and, shortly thereafter, the Plant Manager and the Supply System Managing Director were informed. The licensee is conducting an investigation into the event and will provide additional facts to the NRC on Monday, August 29, 1994.
