MR Number: 3-94-0148 Braidwood 1 Date: 08/19/94 Braceville,Illinois SRI VIA PC Dockets: 50-456 PWR/W-4-LP Subject: UNIT 1 FORCED SHUTDOWN DUSE TO BLOWN RUPTURE DISC Reportable Event Number: N/A Discussion: On August 19, 1994, at 3:15 a.m., while ramping up and at 75 percent reactor power, the Unit 1 heater drain tank rupture disc failed, necessitating a plant shutdown to hot standby to effect repairs. Following the failure, the condenser vent line downstream of the rupture disc cracked, which will also require repairs prior to startup. The licensee estimates the repairs will require the unit to be shutdown about 3 days. The cause of the failure is unknown; however, during a previous startup 6 days earlier, the licensee believed that the rupture disc may have been wetted, which can cause premature failure. This disc was replaced 1 1/2 years ago. No personnel injuries occurred. Regional Action: The resident inspectors will follow the licensee's repair efforts and root cause analysis. Contact: L. Upon notification, the licensee deactivated the sergeant's security badge and suspended him for 14 days while he enters the licensee's employee assistance program. Licensee security management informed the senior resident inspector that there has been no indication that work performance was adversely affected, since the sergeant's activities normally have oversight both from other security personnel and management.
