MR Number: 1-94-0091 Haddam Neck 1 Date: 08/12/94 Hartford,Connecticut SRI E-MAIL Dockets: 50-213 PWR/W-4-LP Subject: LOW TEMPERATURE OVER PRESSURE RELIEF VALVE ACTUATION Reportable Event Number: N/A Discussion: On August 10 at approximately 1:20 p.m., with the unit in a cold shutdown condition, the unit experienced a pressure transient on the reactor coolant system. At the time the licensee was performing a surveillance to measure operational leakage in portions of the charging and residual heat removal system. The surveillance procedure was required pursuant to technical specification 4.4.6.2.1.g. As required by the surveillance, operators started the "A" charging pump. An unwanted leak path not identified in the procedure, existed from the charging system to the reactor coolant pump seal header. The subsequent mass addition into a water solid reactor coolant system (temperature 120 F, pressure 300 psig) resulted in an apparent opening of the low temperature overpressure (LTOP) relief valves. Maximum recorded reactor coolant system pressure was 390 psig. The LTOP relief valves are set at 380 psig to prevent exceeding the 450 psig limit as read off a curve in the technical specifications. The mass addition was terminated by the operator placing the charging pump in "trip pull out." The charging pump was in operation for approximately eighteen seconds. Preliminary licensee investigation concluded that the surveillance procedure was inadequate in that it failed to address complete isolation of the charging system from the reactor coolant system. The licensee was in the process of correcting the procedure, and reviewing past surveillances to understand the basis of the procedural actions. A special report is required pursuant to technical specification 3.4.9.3.c. Regional Action: Investigating the licensee's root cause analysis of this event and basis for performing the surveillance in water-solid condition. MR Number: 2-94-0075 Oconee 3 Date: 08/12/94 Seneca,South Carolina Dockets: 50-287 PWR/B&W-L-LP Subject: OCONEE UNIT 3 TRIP Reportable Event Number: N/A Discussion: At 4:46 a.m., on August 12, 1994, Oconee Unit 3 tripped from 42% power due to the loss of its operating feedwater pump (1B). The unit was being returned to power after a trip that occurred on August 10, 1994, which was caused by a loss of power to the Integrated Control System (See PNO-II-94-042). The unit is currently in Hot Shutdown and the cause of the loss of the feedwater pump is being investigated.
