MR Number: 1-94-0090 Nine Mile Point 2 Date: 08/08/94 Lycoming,New York SRI PC Dockets: 50-410 BWR/GE-5 Subject: LOSS OF ABILITY TO MANUALLY MOVE CONTROL RODS Reportable Event Number: N/A Discussion: At 12:37 a.m. on August 3, 1994, the Nine Mile Unit 2 digital rod drive control system (RDCS) failed, resulting in the inability to manually position control rods. The failure did not affect the reactor protection system or the ability to scram the control rods. Plant conditions were stable at 100% power. After confirming rod position indication was available, and unsuccessfully attempting to reset the RDCS, the operations staff began continuous monitoring of the control rod scram accumulator pressure. This function, normally provided by a scan feature of the RDCS, ensures that accumulator pressures are sufficient to properly scram each control rod when demanded. Subsequent troubleshooting identified a failed transponder card for the hydraulic control unit associated with control rod 46-51. The transponder card was replaced and the card was shipped to the vendor for failure analysis. The RDCS was returned to normal at 6:42 a.m.. Failure of the RDCS did not require immediate entry into a technical specification limiting condition for operation. Throughout this event the recirculation system remained available to reduce reactor power if necessary. Regional Action: None. SCHIN (404)331-5561 PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION IV AUGUST 8, 1994 Licensee/Facility: Notification: Nebraska Public Power District MR Number: 4-94-0076 Cooper 1 Date: 07/28/94 Brownsville,Nebraska SRI Telecon 8/8/94 Dockets: 50-298 BWR/GE-4 Subject: SYSTEM VALVE LINEUPS Reportable Event Number: N/A Discussion: On July 28, 1994, the licensee noted flow indication anomalies between the Reactor Recirculation Pump B flow transmitters and determined that a root valve to one of the flow transmitters was closed. This closure was attributed to an instrument and control technician not following instructions on a clearance order. As a result of this finding, the licensee instituted system lineup reviews that identified a total of approximately 25 additional valves positioned incorrectly. A total of 29 out of approximately 56 system valve lineups have been verified. No further valve lineup checks are planned; however, the licensee is currently performing electrical breaker lineup verifications (for safety and nonsafety-related system).
