MR Number: 2-94-0072 Robinson 2 Date: 08/03/94 Hartsville,South Carolina Dockets: 50-261 PWR/W-3-LP Subject: ROBINSON MANUAL REACTOR SCRAM Reportable Event Number: 27619 Discussion: The unit is in Hot Shutdown following the manual reactor scram on August 2, 1994, at 5:41 a.m. During the scram, the PORVs lifted due to the resultant, expected increase in pressure; however, control room indication showed that one PORV remained open (dual indication). Upon investigation, the licensee determined that the PORV was seated and that the cause of the dual indication was a limit switch problem. The limit switch has been repaired. The cause of the load reject, which prompted operators to manually scram the reactor, is unknown, and the licensee is investigating. It appears that the turbine governor valves closed, but the reason for the closure is unknown. The licensee plans to restart after determination and evaluation of the cause of the load rejection. OF PUBLC HEAL License No: IA 0126-1-57 Subject: DAMAGED MOISTURE/DENSITY GAUGE Reportable Event Number: N/A Discussion: THE IOWA DEPARTMENT OF PUBLIC HEALTH (IDPH) REPORTED THAT TERRACON CONSULTANTS, INC. HAD A MOISTURE/DENSITY GAUGE DAMAGED WHEN RUN OVER BY A BACKHOE ON A CONSTRUCTION SITE IN DAVENPORT, IOWA. THE INCIDENT OCCURRED ON 8/2/94 AT APPROXIMATELY 10:00 A.M. AND INVOLVED A TROXLER 3411B MOISTURE/DENSITY GAUGE CONTAINING 10 MILLICURIE CESIUM-137 AND 50 MILLICURIE AMERICIUM-241 SEALED SOURCES. AN IDPH INSPECTOR RESPONDED TO THE SCENE AND PERFORMED SURVEYS AND FIELD WIPE TESTS OF THE DAMAGED GAUGE. THE GAUGE WAS HEAVILY DAMAGED, HOWEVER, SURVEYS AT THE SCENE INDICATED THAT THE SEALED SOURCES WERE INTACT. THE IOWA LICENSEE IS IN CONTACT WITH TROXLER AND WILL ARRANGE TRANSFER OF THE GAUGE TO THE TROXLER FACILITY. Regional Action: THE REGION 3 STATE AGREEMENTS OFFICER IS MONITORING THE SITUATION. LYNCH (708)829-9818 PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION III AUGUST 3, 1994 Licensee/Facility: Notification: Midwest Metallics (Non-Licensee) MR Number: 3-94-0139 Midwest Metallics (Non-Licensee) Date: 08/02/94 Chicago,Illinois TELECON FROM IL DEPT OF NUCLEAR SAF Subject: CONTAMINATED SCRAP Reportable Event Number: N/A Discussion: The Illinois Department of Nuclear Safety reported that a scrap broker, Midwest Metallics, identified radioactive material in a load of scrap from another Chicago scrap broker, American Steel Foundries. Midwest Metallics personnel were concerned that they may have been contaminated by the material. Two IDNS inspectors responded to the facility with a portable multi-channel analyzer. The radioactive material was identified as a vial of uranyl nitrate and a broken container of ore, identified as uranium and radium. The ground near the container was slightly contaminated from the ore. The contaminated earth was dug up and placed in a steel container with the sources. No appreciable external exposure rate was detected. Surveys of Midwest Metallics employees may have identified a small spot of contamination on one person's wrist, which was easily washed off. The radioactive material was taken back to American Steel Foundries, in a DOT-approved steel overpack, for transfer to a waste broker.
