MR Number: 1-94-0078 Peach Bottom 2 3 Date: 07/14/94 Philadelphia,Pennsylvania SRI PC Dockets: 50-277,50-278 BWR/GE-4,BWR/GE-4 Subject: THREE MAINTENANCE WORKERS DENIED ACCESS DUE TO DRUG USE Reportable Event Number: N/A Discussion: PECO denied access to, and started the dismissal process for, three maintenance workers due to drug use and lack of trustworthiness. On July 11 one worker, who had previously tested positive and was in the employee assistance program, was tested as part of the fitness for duty program. The urine sample did not meet the temperature requirements. An observed sample was subsequently taken and was positive for marijuana. PECO learned through their investigation that the warm sample was from the worker's child and had been in the worker's truck until he went to give the sample. When questioned by PECO security this individual stated that two other maintenance workers were also current drug users or may have provided falsified samples in the past. For one of the two, it was alleged that the worker had a leg prosthesis device in which he could hide a sample, a cigarette lighter, and a digital thermometer (used to assure meeting temperature limits). PECO conducted for cause, witnessed urine sampling of the two other workers on July 12. The sample from the individual with the prosthesis device came back positive for marijuana and metamphetamines. The other individual's sample came back negative after he had admitted marijuana use. PECO used a drug dog to search the individuals' vehicles. In the case of the two cars, PECO identified marijuana and marijuana residue. The individuals, who were already assigned to a group of performers requiring extra supervision, have lost their access to the protected area, which PECO management has said will not be reinstated. PECO continues to follow their policy for fitness for duty and discharge of employees following the identification of drug use or of introduction of drugs or drug paraphernalia onto company property, and has suspended all three individuals with intent to dismiss. Regional Action: The resident staff continues monitoring licensee activities. MR Number: 2-94-0059 Saint Lucie 2 Date: 07/15/94 Florida Dockets: 50-389 PWR/CE Subject: FAILURE OF TRIP CIRCUIT BREAKER (UPDATE) Reportable Event Number: N/A Discussion: St. Lucie Unit 2 was shut down on July 14, to investigate the failure of one of eight trip circuit breakers to trip when called upon during the monthly logic matrix test per Technical Specification 4.2.1.1. The circuit breakers are the GE AK 2-25 series. Investigation by the utility and a GE representative found that a small piece of the cutoff micro switch (phenolic) broke off the corner of the switch. A machine screw attaching the switch to the mounting became loose. Then during breaker operation a small piece (1 1/4" square) broke off and fell into the trip latch areas and prevented the breaker from opening. This switch was installed by the manufacturer during fabrication and was not involved in any licensee maintenance action. All other similar breakers were inspected by the licensee. While other switches had scratches or "mold marks", all were observed under magnification and found not to be significant. Thus the licensee concluded the circuit breaker failure appeared to be a random event. The breaker has been replaced. The unit is in the process of restarting as of 9:00 a.m. on July 15. LONG (404)331-4664 PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION II JULY 15, 1994 Licensee/Facility: Notification: Tennessee Valley Authority MR Number: 2-94-0060 Sequoyah 1 2 Date: 07/15/94 Soddy-Daisy,Tennessee Dockets: 50-327,50-328 PWR/W-4-LP,PWR/W-4-LP Subject: TURBINE GENERATOR SHUTDOWN TO REPAIR GENERATOR VOLTAGE REGULATOR Reportable Event Number: N/A Discussion: On July 14, at approximately 10:00 p.m., Sequoyah, Unit 1 commenced a Unit power reduction in order to remove the generator from the grid and make repairs to the voltage regulator. The licensee identified problems in one of the voltage regulator power drawers which required that the generator be off line to repair. Unit 1 reactor power will be maintained at approximately 15% during the repair period. The repairs are expected to take less than two days. As of 7:30 a.m., on July 15, Unit 1 was at approximately 30% power. Disconnect of the generator from the grid is estimated for 10:00 a.m. Unit 2 is currently in day 11 (Mode 6) of a refueling outage which is expected to last until early October 1994. Regional Action: The Resident Inspectors are monitoring licensee Unit 2 refueling and Unit 1 shutdown forced outage activities. Contact: M. MR Number: 3-94-0127 Quad Cities 1 Date: 07/15/94 Cordova,Illinois SRI VIA LAN Dockets: 50-254 BWR/GE-3 Subject: BLOCKAGE OF RHR TORUS COOLING RETURN LINE DUE TO FOREIGN MATERIA Reportable Event Number: N/A Discussion: During a post maintenance test run of the A loop of RHR, a maximum flow of about 2000 gpm at 240 psi was achieved. Normal flow is about 4500 gpm at 180 psi. Test data indicated that valve 1001-36A (RHR Torus Cooling/Test Return Valve) was plugged. Anti-cavitation trim, which provides a tortuous flow path through the valve, had just been installed in the 36A valve during the current refuel outage. When the valve was opened, the remains of a yellow plastic bag were found shredded and caught within the valve trim. Very little bag material appeared to have travelled the entire way through the trim. The majority of the material was lodged on the upstream side of the valve trim. The reactor is in cold shutdown nearing completion of a 4 month refueling outage. The reactor head is still removed, fuel is in the vessel, and shutdown cooling is in service. The RHR torus cooing and spray function were not required to be in service at the time of the foreign material discovery. The licensee verified that shutdown cooling flow was not affected by flow blockage. The licensee is investigating the source of the bag and inspecting other locations where the material might have gone. Visual inspections of the 36A valve body and boroscope inspections of adjacent upstream and downstream piping so far showed no other signs of foreign material. Regional Action: The resident inspectors were onsite to observe the licensee's testing and inspection activities. The inspectors found metallic shavings, apparently from valve lapping, also lodged within the valve trim. Resident and regional inspectors will continue to evaluate the effects of foreign material on safety system performance, and pursue the effectiveness of licensee corrective actions. Contact: P.L.
