MR Number: 1-94-0074 Indian Point 2 Date: 06/30/94 Buchanan,New York SRI Dockets: 50-247 PWR/W-4-LP Subject: SERVICE WATER PUMP FAILURE POSSIBLY DUE TO SHAFT FAILURE Reportable Event Number: N/A Discussion: On June 29, 1994 while operating at 90% power, the #21 service water pump failed to develop discharge pressure during a pump start. Scheduled preventive maintenance had just been completed on the service water strainer associated with that pump and the pump was being started for post maintenance testing. There have been three recent service water pump failures at Indian Point 2. In August 1993, two pumps failed due to sudden impact loading caused by foreign object ingestion. (reference reportable event #25902). In September 1993, a pump failure was attributed to impact loading caused by reverse flow through the check valve during startup. In all cases, a contributing cause was the low impact resistance of the material used for the pump shaft couplings. All three previous pumps' failures involved pumps that were manufactured by Aurora. The pump which just failed was manufactured by Layne and Bowler. The pump which failed was in the designated non-essential service water header. Technical specifications for the non-essential header require two of the three pumps to be operable. All other service water pumps are operable. Regional Action: Residents to followup. INSPECTOR 6/29/94 Dockets: 50-348,50-364 PWR/W-3-LP,PWR/W-3-LP Subject: LIFTING CRANE OVERTURNS WITH SPARE REACTOR COOLANT PUMP MOTOR Reportable Event Number: N/A Discussion: While lifting a spare reactor coolant pump motor (RCP) frame with rotor (no stator) in a vacant field outside the perimeter area on June 29, 1994, around 11:00 a.m. (CST) in order to return it to the warehouse, the lifting crane overturned when its cribbing moved and the spare RCP motor fell to the ground. Damage was believed to be neglible; however, the licensee is considering shipping RCP motor back to Westinghouse for evaluation. There were no personnel injuries. MR Number: 3-94-0120 Quad Cities 2 Date: 06/30/94 Cordova,Illinois E-MAIL Dockets: 50-265 BWR/GE-3 Subject: NOTIFICATION OF UNUSUAL EVENT Reportable Event Number: 27466 Discussion: The Unit 2 generator was taken off line on June 29, 1994, at 3:45 a.m. (CDT) due to increased acetylene in the Unit 2 reserve auxiliary transformer (RAT) 22. The increase was observed by the system engineer and verified by an oil sample to be about 41 ppm acetylene. The RAT's acetylene concentration continued to increase until removed from service at approximately 10:30 p.m. to avoid possible damage to the windings. The licensee did non de-energize RAT 22 until completion of the electrical lineup to supply offsite power to the 4160 volt busses for Unit 2 through cross-tie breakers from Unit 1 (which is shut down for refueling). Loss of both transformers, unit auxiliary transformer (UAT) 21 due to shutdown and RAT 22, required entry into Emergency Action Level MU1 (loss of offsite power sources), and notification of unusual event (NOUE). The licensee commenced efforts to supply a temporary backfeed through Unit 2's main transformer and UAT 21. The backfeed is expected to be in service the afternoon of June 30, at which time the NOUE will be exited. Regional Action: Resident and regional inspectors were onsite to observe bus manipulations and follow the transformer troubleshooting activities. Contact: P.L. MR Number: 4-94-0062 Palo Verde 1 2 Date: 06/29/94 Wintersburg,Arizona Licensee Telecon Dockets: 50-528,50-529 PWR/CE80,PWR/CE80 Subject: UNITS 1 AND 2 INCREASE REACTOR POWER TO 100 PERCENT Reportable Event Number: N/A Discussion: On June 29, 1994, Arizona Public Service Company (APS) informed the Region IV Walnut Creek Field Office of their intention to increase reactor power in Palo Verde Units 1 and 2 to 100 percent. This action was being taken as a result of decreased grid margins due to high temperatures and high grid loading in the Southwestern states. Previously, Palo Verde Units 1 and 2 had been operating at 86 percent power to lower reactor coolant temperature in an attempt to reduce corrosion in steam generator tubes. At this time, APS intends to continue plant operation at 100 percent power until the grid margins are regained (possibly through the weekend). Palo Verde Unit 3 is currently operating at 100 percent power. Unit 3 had accomplished plant modifications, not yet complete in Units 1 and 2, which allow full power operation at reduced reactor coolant temperatures. Regional Action: Discussions were held among the licensee, Region IV management, and NRR to assure that there were no safety or technical issues associated with increasing power in Units 1 and 2.
