MR Number: 2-94-0047 Turkey Point 3 Date: 06/16/94 Miami,Florida Dockets: 50-250 PWR/W-3-LP Subject: UNIT 3 LOAD REDUCTION Reportable Event Number: N/A Discussion: At 9:50 p.m. on June 13, 1994, with Unit 3 at 100% power, control room operators noted a slight decrease in main generator output followed by a rapid drop to 70% reactor power and a trip of both heater drain pumps. Operators started the third condensate pump, restarted one heater drain pump, and opened the low pressure feedwater heater bypass valve in order to ensure adequate feedwater pump suction pressure. Operators noted that the northeast intercept valve was closed resulting in the isolation of steam from the 3C moisture separator reheater to the B low pressure turbine. Further investigation revealed a break of the control oil supply pipe to the northeast intercept valve which caused the valve to fail close per design. The licensee isolated the oil supply line and the control oil leak was contained by the designed "guard" pipe. The control oil line was found to be completely sheared at the valve servo. This was apparently due to a fatigue failure in the heat affected zone of the pipe weld. This 3/4 inch carbon steel pipe operates at a pressure range of 25 psig to 50 psig. Similar failures occurred in 1991, and the licensee performed modifications to support this piping. The licensee intends to operate the unit at a reduced load per Westinghouse recommendations until repairs are completed. An on line repair of the control oil line is currently planned. Contact: KERRY LANDIS (404)331-5509 PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION II JUNE 16, 1994 Licensee/Facility: Notification: Veterans Affairs Medical Center MR Number: 2-94-0048 Veterans Affairs Medical Center Date: 06/15/94 Jackson,Mississippi By Telephone Dockets: 03002261 License No: 23-08786-01 Subject: UNINTENDED EXPOSURE TO A FETUS Reportable Event Number: N/A Discussion: On June 15, 1994, the licensee notified Region II that on May 4, 1994, a female patient had been administered 10.5 millicuries of I-131 for therapy. Prior to the administration, the licensee questioned the patient concerning her pregnancy status. The patient indicated that she was taking birth control pills and was not pregnant. On May 25, 1994, the patient notified the licensee that she was pregnant. Ultrasound tests performed on June 3, 1994, indicated that the patient was approxiamtely five weeks pregnant at the time the I-131 dosage was given. The licensee has estimated that the total dose to fetus from the administration will be 378 mrem.
