The SRO had come off shift earlier that day and did not display symptoms of alcohol while on duty. The SRO was removed from duty and has voluntarily entered a rehabilitation program. The licensee intends to perform a medical evaluation prior to his returning to duty. This individual tested positive for alcohol on a random test several years ago. MR Number: 3-94-0109 Quad Cities 1 2 Date: 06/09/94 Cordova,Illinois LICENSEE PHONE CALL Dockets: 50-254,50-265 BWR/GE-3,BWR/GE-3 Subject: UNEXPECTED EXPOSURE Reportable Event Number: N/A Discussion: On June 8, 1994, at about 8:30 p.m., a contract worker was moving a bucket in the spent fuel pool (via a rope tied to the handle) when it apparently came too close to the surface causing both his and another worker's electronic dosimeters (EDs) to alarm on high dose rate. No administrative or regulatory dose limits were exceeded and the highest dose rate recorded by the EDs was about 170 millirem/hr. The bucket contained irradiated components that had been stored in the pool since May 20, 1994. The licensee stated that the contact dose rate could be between 15,000 and 20,000 rem/hr, based on historical data. The root cause appears to be an inadequate procedure and personnel error. Although the bucket was on the spent fuel pool inventory, the inventory is not required to be reviewed prior to relocating stored items. Additionally, a radiation protection technician on the refuel floor may not have been notified of the move. Corrective actions included stopping all refuel floor activities pending further review by licensee management. Regional Action: A regional radiation specialist will review the event onsite.
