The shutdown is currently scheduled to end on June 26, 1994. Activities to be included during the shutdown include cavity seal ring repair, modifying the pressurizer safety valves from a water seal to a steam environment, and several steam system general repairs. ON JUNE 4, 1994, LIGHTNING STRUCK THE EXHAUST STACK AT MONTICELLO AND CAUSED THE LOSS OF TWO PRESSURE TRANSMITTERS AND SEVERAL RECTIFIERS IN THE OFFGAS SYSTEM. THE TRANSMITTER FAILURES CAUSED A TRIP OF THE RUNNING "A" TRAIN OF OFFGAS AND OPENING OF THE DUMP VALVE FROM THE OFFGAS TRAIN BACK TO THE CONDENSER. OPERATORS ATTEMPTED TO RESTORE THE OFFGAS SYSTEM BY REDUCING POWER AND INITIATING THE "B" TRAIN; HOWEVER, THEY COULD NOT REOPEN THE DUMP VALVE QUICKLY ENOUGH TO PRECLUDE CONDENSER VACUUM REACHING THE TURBINE TRIP POINT. THE REACTOR TRIPPED ON TURBINE TRIP AND ALL SYSTEMS FUNCTIONED NORMALLY. COOLDOWN WAS VIA THE CONDENSER AND THE MAIN FEEDWATER SYSTEM. THE PLANT COMMENCED A RESTART ON JUNE 5, 1994; HOWEVER, ADDITIONAL TUBE LEAKS IN A HEATER DRAIN COOLER WERE IDENTIFIED DURING THE STARTUP. GIVEN THESE ADDITIONAL TUBE LEAKS, THE LICENSEE DECIDED TO SHUT THE PLANT DOWN AND COMMENCE THE FIVE-DAY MAINTENANCE OUTAGE ORIGINALLY PLANNED TO BEGIN ON JUNE 10TH. CLAYTON (708)829-9602 PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION III JUNE 6, 1994 Licensee/Facility: Notification: Alliant Tech Systems MR Number: 3-94-0106 Joliet Army Ammunition Plant Date: 06/06/94 Wilmington,Illinois PHONE CALL TO NRC HEADQUARTERS Subject: CONTAMINATION FOUND AT PREVIOUSLY LICENSED FACILITY Reportable Event Number: N/A Discussion: Alliant Tech Systems notified the NRC on June 1, 1994, that contamination was found at the previously licensed Joliet Army Ammunition Plant (JAAP) in Wilmington, Illinois. JAAP is a federal facility and is used for the testing of munitions. Alliant fired depleted uranium (DU) projectiles into sand traps in an enclosed firing range until mid-1989. From October 1989 to February 1990, the DU was removed from the site. It has been discovered that a pile of approximately 100 cubic yards of sand contains spent DU projectiles. The pile is apparently the result of having removed sand from the ranges as it became pulverized. It is not known why this sand was not part of the original site remediation. Over the past two months, some of this sand has been placed into one or two of the ranges, possibly contaminating them. A roadway near the pile is slightly contaminated as well. These areas have been roped off. Initial surveys indicated that levels of contamination were low. As a precaution, urine samples were planned for employees who may have come in contact with the contaminated sand. Alliant has contracted with SEG Services of Oak Ridge, Tennessee to conduct characterization surveys and remediate the site. Regional Action: The Region will conduct a special inspection of the site on June 9, 1994. NMSS and the State of Illinois have been notified. Date: 06/06/94 Dayton,Ohio TELECON Dockets: 03012676 Subject: STOLEN MOISTURE-DENSITY GAUGE Reportable Event Number: N/A Discussion: On June 2, 1994 at 6:54 p.m. (ET), the licensee informed the NRC Headquarters Operations Officer that a moisture-density gauge, manufactured by Troxler, Inc, (Model No. 3401) had been stolen from a locked camper vehicle parked in a bowling alley parking lot in Dayton, Ohio. According to the licensee's RSO, the gauge source rod was locked and the device was locked in the manufacturers shipping container. The gauge was discovered missing at approximately 6:00 p.m. (EDT) on June 2, 1994. The gauge contains a nominal 8 millicurie cesium-137 sealed source and a 40 millicurie americium-241 sealed source. In the shielded position, the radioactive materials do not present a significant radiation hazard. With the gauge stored in the shipping container, the maximum radiation level at the surface of the shipping container is approximately 6 mR/hr (1.5 micro coulombs per kilogram per hour). The licensee notified the local police department and an investigation of the theft has begun. The police issued a press release. At approximately 3:30 p.m. (CDT) on June 3, 1994, the licensee notified the NRC Region III that the gauge had been recovered by a waste collection department employee near a dumpster approximately 5 miles from where it had been stolen. The worker returned the device to the licensee. Preliminary surveys performed failed to indicate abnormal readings. The licensee stated that the shipping case padlock and hasp had been broken. The padlock attached to the source rod, however, was not removed or tampered with and there appeared to be no damage to the gauge. Preliminary leak test results indicate no removable contamination. The licensee submitted a written report to Region III (Chicago) for evaluation via fax dated June 6, 1994. Region III will review the circumstances surrounding the theft during a future inspection. Clarke, Chairman and CEO of PG&E, announced on June 2, 1994, that effective July 1, 1994, Stanley Skinner, President and Chief Operating Officer of PG&E, will become the President and CEO. Mr. Skinner will assume overall management of the company, including the functions that have been reporting to Mr. Clarke. Mr. Clarke will remain Chairman of the Board until his mandatory retirement in May of 1995. PG&E also announced two other senior management changes to take effect July 1, 1994. Robert D. Glynn, Jr., Senior Vice President and General Manager of the customer energy services business unit, has been promoted to Executive Vice President in charge of the utility business units formerly reporting to Mr. Skinner.
