MR Number: 1-94-0062 Oyster Creek 1 Date: 06/02/94 Forked River,New Jersey SRI PC Dockets: 50-219 BWR/GE-2 Subject: PLANT TRIP DUE TO SWITCHYARD WORK Reportable Event Number: 27323 Discussion: At 1:30 p.m., May 31, 1994, the reactor automatically scrammed from full power. A Jersey Central Power & Light (JCP&L) relay technician was installing a digital fault recorder in the substation 230 kilovolt differential fault circuit at the time of the scram. While disconnecting a terminal to connect the recorder, a momentary current path was established through the differential fault relay which tripped the turbine generator, resulting in a reactor scram. The work being performed was on a de-energized relay, however, an electrical push existed through a lamp circuit. Actuation of the relay was duplicated during subsequent investigation to determine the cause of the scram. All systems functioned normally on the reactor scram. All maintenance and surveillance activities in the substation are performed by JCP&L. Under agreement (between GPUN and JCP&L) the control room is to be notified prior to starting any work in the substation with a brief description of activities and notified again when they depart. The control room supervisor had been notified as required. GPUN and JCP&L held a meeting on June 1, 1994, to discuss this event and to strengthen their agreement such that site (GPUN) engineering is also informed of the specific activities to be performed in the substation prior to their performance. On June 2, 1994, GPUN discussed the enhancements to the agreement and station work control practices. Regional Action: Regional management conference call with GPU. Resident inspector followup. MR Number: 4-94-0051 Arkansas Nuclear 1 Date: 06/01/94 Russelville,Arkansas PHONE CALL FROM HOO Dockets: 50-313 PWR/B&W-L-LP Subject: LICENSEE EMPLOYEE TESTS POSITIVE FOR DRUGS Reportable Event Number: 27327 Discussion: On June 1, 1994, Arkansas Nuclear One reported that a positive test for marijuana was identified during a random drug screen of a licensee senior Instrumentation and Controls technician who occasionally served as a temporary department supervisor. This individual had access to both protected and vital areas in the facility. Plant access for the individual has been terminated until a further review is completed. A quality review of the individual's work has been conducted. Regional Action: Regional based inspectors will perform followup during the next routine inspection.
