MR Number: 1-94-0054 Beaver Valley 1 Date: 05/09/94 Shippingport,Pennsylvania RI PC Dockets: 50-334 PWR/W-3-LP Subject: UNPLANNED SHUTDOWN OF GREATER THAN 48 HOURS DUE TO A RIVER WATER SYSTEM LEAK Reportable Event Number: 27218 Discussion: At 9:20 p.m. on May 6, 1994, Beaver Valley Unit 1 commenced a plant shutdown due to a leak on the `A' train reactor plant river water (RPRW) system. The shutdown was initiated prior to the expiration of the 72-hour technical specification limiting condition for operation requiring two operable river water subsystems. The `A' RPRW train was previously declared inoperable on May 4 when water was identified leaking past a piping penetration in the primary auxiliary building. Excavation efforts have located a 1/2-inch diameter hole on the buried portion of the `A' river water supply line to the emergency diesel generators. This piping is 6-inch diameter carbon steel piping rated at 85 psig. The failure mechanism is currently under investigation as nondestructive examinations continue. Preliminary results have identified wall-thinning in the vicinity of the hole. The `B' RPRW train remains operable as a source of cooling water to both emergency diesel generators. The plant entered Mode 3 on May 7 at 9:18 a.m. and is currently stable in cold shutdown. The outage duration is currently scheduled for 7 days. Regional Action: The NRC resident inspectors and a region based inspector are monitoring the licensee's efforts to evaluate and repair the source of the river water leak. The main generator was disconnected from the grid at 9:19 p.m. (CDT) and the reactor trip breakers were opened at 11:06 p.m. In addition to refueling, major work will include: 10-year inservice inspection of the reactor vessel, steam generator eddy current testing, completion of safeguards electrical systems upgrade modification, completion of control room panel human factors upgrade modification, containment integrated leakrate testing, and Generic Letter 89- 10 motor-operated valve testing. Major scheduled work accomplished during the outage included installation of the EAGLE 21 solid state reactor protection system and motor-operated valve testing pursuant to Generic Letter 89-10. The plant is currently holding at 30 percent power pending completion of core physics testing. The licensee expects to achieve full power by May 12, 1994.
