MR Number: 3-94-0090 Dresden 2 Date: 05/02/94 Morris,Illinois LAN Dockets: 50-237 BWR/GE-3 Subject: U-2 SCRAM WITH COMPLICATIONS Reportable Event Number: 27193 Discussion: Unit 2 was manually scrammed at approximately 2335 CDT on April 30, 1994. The reactor was placed in cold shutdown. The reason for the scram was low instrument air pressure resulting from the separation of the 2A instrument air compressor discharge line to the instrument air receiver. The cause of the separation was unknown but appears to be a combination of corrosion and vibration. The 2B instrument air receiver was inspected. Thinning of the pipe wall was found but considered acceptable. The major complication resulting from the scram was reactor water level reached +65 inches by wide range digital indication. The high pressure core injection (HPCI) steam nozzle was at +62 inches. The HPCI supply and exhaust drain pot high level alarms were received; and stayed in alarm for approximately 38 minutes. The licensee did not declare the HPCI system inoperable, because it was designed to operate with water in the system. The reason for the high reactor water level was unknown. The licensee event notification indicated that the 2A feed water regulating valve was in manual and not shut. The operator at the feedwater control panel stated the 2A feed water regulating valve was shut at +2 inches. The licensee continues to investigate the root cause of this event.
