MR Number: 1-94-0043 Nine Mile Point 1 Date: 04/05/94 Oswego,New York SRI PC Dockets: 50-220 BWR/GE-2 Subject: REACTOR SCRAM DURING SWITCHYARD WORK PREPARATIONS Reportable Event Number: N/A Discussion: At 4:52 am and 100% power, Nine Mile Unit 1 experienced a turbine trip and accompanying reactor scram from a main generator load rejection. During preparations to repair a disconnect switch in the 345 kV switchyard, one of the two main generator supply breakers to the 345 kV ring bus was opened. Coincident with the planned opening of this breaker, the other breaker supplying the ring bus unexpectedly opened. This resulted in a complete load rejection and scram. The cause of the unexpected opening of the supply breaker is unkown. All systems functioned as expected with the exception of the position indication of one control rod. Unit 1 is proceeding to cold shutdown. Major work planned for the approximately 1 week forced outage includes final installation of the reactor vessel water level modification (Bulletin 93-03). MR Number: 3-94-0070 Big Rock Point 1 Date: 04/04/94 Charlevoix,Michigan RESIDENT OFFICE TELECON Dockets: 50-155 BWR/GE-1 Subject: PLANT SHUTDOWN IN EXCESS OF 72 HOURS Reportable Event Number: N/A Discussion: AT 2:07 P.M. EST THE PLANT STARTED TO SHUTDOWN IN ORDER TO REPAIR SEVERAL ITEMS THAT HAD NOT BEEN SATISFACTORILY REPAIRED DURING THE MARCH MAINTENANCE OUTAGE, INCLUDING AN INCREASING STEAM LEAK ON THE VFW-24 HIGH PRESSURE FEEDWATER HEATER CONTROL VALVE. THE PLANT WAS DISCONNECTED FROM THE GRID AT 5:12 P.M. EST, AND REACHED COLD SHUTDOWN AT 6:18 A.M. EST THIS MORNING. REPAIRS ARE EXPECTED TO TAKE APPROXIMATELY SEVEN DAYS. OTHER MORE SIGNIFICANT REPEAT MAINTENANCE WORK SCHEDULED FOR THIS OUTAGE INCLUDES REPAIR OF THE TURBINE'S INTERNAL PRESSURE REGULATOR AND SYNCHRONOUS GOVERNOR, WHICH HAD LIMITED THE LICENSEE TO 85 PERCENT POWER, AND REPAIRS OF LEAKS ON THE "D" REACTOR DEPRESSURIZATION SYSTEM LINE.
