MR Number: 4-94-0032 South Texas 1 2 Date: 04/19/94 Wadsworth,Texas Licensee telecon 4/18/94 Dockets: 50-498,50-499 PWR/W-4-LP,PWR/W-4-LP Subject: STANDBY DIESEL GENERATOR FUEL INJECTION PUMP STUD FAILURE Reportable Event Number: N/A Discussion: On April 14, 1994, during a postmaintenance test run on Standby Diesel Generator 22, the studs on the Fuel Injection 6L pump failed. The operator in the diesel generator room shut down the engine. Subsequent testing and analysis by the licensee has not been successful in establishing a definite root cause of the failure. The licensee is processing a design change to replace the hollow studs with solid studs. The licensee has initiated a test program to help establish the reason for this engine's history of stud failures. Extra vibration monitoring has been installed on all of the Unit 2 SDGs to monitor vibration during low speed operation and operation at various loads. On April 19, testing of SDG 22 was underway, to be followed by testing the other two engines. Regional Action: The Region is monitoring the licensee's actions and participating in conference calls with the licensee and NRR. Contact: Bill Johnson (817)860-8148 PRIORITY ATTENTION REQUIRED MORNING REPORT - REGION IV APRIL 19, 1994 Licensee/Facility: Notification: Nebraska Public Power District MR Number: 4-94-0033 Cooper 1 Date: 04/19/94 Brownsville,Nebraska Senior Resident Inspector Dockets: 50-298 BWR/GE-4 Subject: CONTROL ROOM EMERGENCY FILTER SYSTEM Reportable Event Number: N/A Discussion: On April 18, 1994, Region IV granted a Notice of Enforcement Discretion (NOED) to the licensee to extend the shutdown action statement time for CNS Technical Specification LCO 3.12.A.3, "Control Room Emergency Filtration System," an additional 14 days to continue troubleshooting and testing activities. The 7-day action statement was entered on April 11, 1994, after CNS Surveillance Procedure 6.3.17.18, "Control Room Envelope Pressurization Test," failed to meet its acceptance criteria following preventive maintenance on the latch for Door 300. The surveillance test was re-performed on April 12, but again failed. The action statement time limit would have expired at 1:47 p.m. on April 18, 1994. The licensee initiated enforcement discretion discussions with Region IV and NRR at 7 a.m. on April 18, 1994. The licensee requested 14 days to continue troubleshooting, repair, and testing of the control room envelope. At 12:30 p.m. Region IV granted the NOED based on compensatory measures taken by the licensee and the low safety significance of the extended period.
