MR Number: 1-94-0046 Hope Creek 1 Date: 04/14/94 Hancocks Bridge,New Jersey SRI PC Dockets: 50-354 BWR/GE-4 Subject: LOSS OF WATER FROM THE SPENT FUEL POOL Reportable Event Number: N/A Discussion: At 10:10 a.m. on 4-13-94, a Hope Creek operator noticed decreasing level in the fuel pool cooling surge tank. Water from skimmers in the spent fuel pool normally drains into the surge tank. The spent fuel pool cooling pump draws water from the surge tank, pumps it through a heat exchanger for cooling, and returns it to the spent fuel pool. Control room operators began taking actions to identify and correct the cause of the unexpected level decrease. At 10:25 a.m. the operators received a report that water was leaking from the spent fuel pool past the gate seals into the reactor well surrounding the containment dome. The spent fuel pool cooling pump tripped due to low water level in the surge tank. Operators established a supply of water from the condensate storage tank to the surge tank, and restarted a spent fuel pool cooling pump. Plant staff discovered that a normally open valve supplying air to the spent fuel pool gate inflatable seals had been closed. The valve was reopened, to inflate the gate seals and at 10:38 a.m. water stopped leaking from the spent fuel pool. At 11:00 a.m., normal level was restored in the spent fuel pool. During the period of leakage the licensee estimates that operators pumped approximately 20,000 gallons of water from the condensate storage tank during the course of the leak. The licensee believes that approximately 15,000 of the 20,000 gallons was pumped into the spent fuel pool. It appears that more than 22 feet of water remained above the top of the spent fuel pool throughout the duration of the leak. On the morning of 4-14, the licensee was in the process of draining water from the reactor well. Regional Action: A teleconference between the licensee and interested NRC organizations was held on 4/13/94. Based on that discussion, NRC staff is satisfied with the licensee's followup activities. The resident inspector is assessing licensee activities and investigation of the cause of this occurrence. (PN-3-9-024). ON APRIL 12, 1994, REGION III LEARNED THROUGH A TELEPHONE CALL FROM THE WISCONSIN DEPARTMENT OF HEALTH THAT A METALS DEALER, "WABASH ALLOYS" IN OAK CREEK, WI HAD RECEIVED A SHIPMENT OF SCRAP FROM "CONSUMERS SCRAP RECYCLING OF DETROIT" IN DETROIT, MI WHICH HAD ALARMED THEIR RADIATION DETECTOR PORTAL MONITORS. AFTER RADIATION SURVEYS BY THE WISCONSIN DEPARTMENT OF HEALTH SHOWED THE TRUCK CONTENTS TO READ 4 MILLIREM PER HOUR (40 MICROSIEVERTS) AT THE SIDES AND 1 MILLIREM PER HOUR (10 MICROSIEVERTS) AT THE TOP, WABASH ALLOYS REFUSED TO ACCEPT THE SHIPMENT AND THE VEHICLE RETURNED TO CONSUMERS SCRAP. THE MICHIGAN DEPARTMENT OF HEALTH MONITORED THE SEARCH OF THE VEHICLE AND FOUND THAT THE RADIATION WAS DUE TO A DAMAGED MOISTURE DENSITY GAUGE. THE SERIAL NUMBER SHOWED IT WAS THE MISSING GAUGE FROM SOIL & MATERIALS ENGINEERS, INC.(S&MEI). S&MEI RECOVERED THE GAUGE AND IS ARRANGING FOR LEAK TESTS TO BE PERFORMED BY THE GAUGE VENDOR. THE TROXLER MODEL 3411B GAUGE CONTAINS A NOMINAL 8 MILLICURIE (3E8 Bq) CESIUM- 137 SOURCE AND A 40 MILLICURIE (14.8E8 Bq) AMERICIUM-241 SOURCE. THE RADIATION SURVEYS BY THE WI AND MI DEPARTMENT OF HEALTHS SHOWED NO LEAKAGE OR CONTAMINATION AND DIRECT READINGS DID NOT POSE AN IMMANENT HEALTH AND SAFETY RISK TO THE PUBLIC. CDT on April 12, 1994. Normal water level is approximately at the 691 foot elevation. The licensee's off-normal procedures required them to commence a controlled shutdown. They began the power reduction at 6:15 P.M. Additionally, as a precautionary measure, the licensee placed sandbags around the circulating water pumps and other equipment at the lake screen house. Water level peaked at 696 foot 2 inches at approximately 9:00 P.M. and began to decrease at approximately 10:00 P.M. At 3:00 A.M. on April 13, 1994, the level decreased below the 696 foot level. Since the weather forecast did not predict additional rain for the region and the water level continued to decrease, the plant was returned to 100 percent power. Mr. John B. Hosmer, formerly Engineering Manager at the Florida Power and Light's St. Lucie station will become Engineering Vice President effective May 2, 1994. He will be responsible for Division engineering functions including establishing policy and setting standards. Mr. Hosmer also has prior experience with the Navy Nuclear Program and with Bechtel Power Corporation. Mr. John "Jack" Brons, formerly President and Chief Operating Officer for the New York Power Authority, will become Executive Vice President for Nuclear Generation, also effective May 2, 1994. He will be responsible to develop and lead the nuclear support organization within the Nuclear Operations Division.
